The perinephric space is a cone-shaped retroperitoneal compartment containing the kidney, adrenal gland, perinephric fat, fibrous bridging septa, and a rich network of perirenal vessel and lymphatics. Perinephric space pathology may originate from within or outside the confines of the perirenal fascia.

Is Perinephric the same as Perirenal?

There are two fat tissue layers covering the kidneys. The more superficial layer is called the perinephric (perirenal) fat lies between the renal fascia and the renal capsule and the deeper fat tissue layer which is also referred as the paranephric fat lies superior to the renal fascia.

Where is the renal space?

The space is surrounded by the perirenal fascia and is in continuity with the opposite perirenal space across the midline. It abuts the bare area of the liver on the right and the subphrenic space on the left; there is mediastinal communication via the various diaphragmatic hiatus.

Spontaneous perirenal hematoma is a rare condition. The clinical features are acute flank or abdominal pain, haematuria, hypotension and shock. Bleeding is most commonly caused by renal tumours, especially angiomyolipomas. Other known causes are long-term haemodialysis, arteriosclerosis or arteritis.
